Plus Plan Apply

Please read the following information:

In order to be covered, your spouse (or common-law spouse) and all eligible dependents must be listed on the application. If applying for the Plus Plan, you will not be required to complete the medical questionnaire portion of the application. If you are a new resident to Manitoba, or have recently lost benefits with an employer group, please contact our office as enrollment procedures may differ.

Please Note: If you or any dependents are covered for prescription drugs through another insurance company, please provide details on the application.
